By accessing our website, scheduling services, or utilizing our services, you agree to the following Terms & Conditions.
1. ServicesPink Mobile Labs & Diagnostics provides mobile and in-office healthcare-related support services including, but not limited to:
Mobile lab collections
Diagnostic testing coordination
EKG services
Mobile X-rays and ultrasounds
IV hydration therapy
Post-operative nursing support
Wellness services
Concierge healthcare coordination
Services are subject to provider availability, clinical appropriateness, and operational coverage areas.
2. Patient Responsibilities
Patients are responsible for:
Providing accurate demographic and medical information
Providing valid physician orders when required
Completing all required intake and consent forms
Providing accurate insurance information
Maintaining a valid payment method on file when applicable
Following pre-appointment instructions
Failure to provide accurate information may result in delays, rescheduling, or cancellation of services.
3. Scheduling & Payments
Payment may be required prior to scheduling services.
Certain services require deposits or full payment in advance.All invoices must be paid by the due date listed.
Mobile services may include travel or convenience fees.
Additional fees may apply for STAT, after-hours, holiday, or last-minute bookings.
4. Cancellation & No Refund Policy
Pink Mobile Labs & Diagnostics maintains a strict No Refund Policy.
Once services are scheduled, staffed, coordinated, or rendered, payments become non-refundable.
In certain circumstances, management may elect to issue:In-store credit
Rescheduling accommodations
Service credits
Missed appointments, patient no-shows, inaccessible locations, incomplete documentation, or late cancellations may result in forfeiture of deposits or appointment fees.
5. Arrival Window
For mobile appointments, patients acknowledge and agree to a service arrival window of up to two (2) hours unless otherwise specified in writing.
6. Safety & Home Environment
Patients receiving mobile services must ensure:
Safe entry access for staff
Pets are secured prior to arrival
A clean and safe environment for staff members
Appropriate adult supervision if medically necessary
Pink Mobile Labs & Diagnostics reserves the right to discontinue or refuse services if staff safety is compromised.
7. Medical Disclaimer
Pink Mobile Labs & Diagnostics does not replace emergency medical services.If you are experiencing a medical emergency, call 911 immediately or seek emergency medical care.
